Companion Community Health Nursing team assists participants and their families to manage various health conditions. The nurses aim to support and promote participant independence, safety, and a healthy lifestyle. In addition, the nurses provide personalised care specific to participant needs. Companion Nurses provide a Person-Centred approach while caring for our participants. This means that the participant decides the nursing support they require and how they are provided.
