Support Coordination

Companion Support Coordinators will assist you to understand the aspects of your plan, assist in ongoing management of supports, and answer questions as they arise. Our objective is to establish your skillset and to help you make the most informed decisions about how to effectively use your funding. The team connects clients with providers for each service.

Companion offers all levels of support coordination including level 3 which provides participants with complex needs or high levels of risks. The specialist team manages complex barriers impacting your ability to implement your plan and access appropriate support. Our team will assist you to reduce complexity in your support environment and overcome barriers with a view to connecting you with broader systems of supports as well as funded supports.

Companion Mental Health and Disability Services is a registered NDIS provider in Perth WA. Companion provides excellent supports to individuals living with Disability.

Our services are Person Centred, that therefore means that the services depend on your specific needs in your own home or one of our accommodations across the city. Our services range from: Assistance with activities of daily living, Community Nursing, Supported Independent Living, Group/Centre Activities, Specialised Disability Accommodation (SDA), Medium Term Accommodation, Short Term Accommodation/Respite, Support in the home and Assistance with Disability Travel / Transport. Companion has friendly, trained professionals and supports workers committed to providing excellent services offered at individual level and aimed at promoting client independence.
